Obtaining
Copies of MAIS (Ambulance Reports)
A copy of an ambulance report may be obtained by contacting the administrative
offices of the Department of Emergency Services during regular business hours
(Monday-Friday, 8:30 am to 5:00 pm). MAIS reports are confidential medical
records. As such, MAIS reports are only released upon authorization of the
patient, their legal representative, or as required by subpoena. To obtain
a copy of a MAIS report, contact:
